Instructions for Forms

All forms are to be signed in the same manner as you normally sign your legal name.  Please call (608) 276-8732 x261 if you have any questions.

Following is miscellaneous information on repeated questions and errors on several of the personnel employment forms. Hopefully this will answer some of the questions frequently asked and save you the inconvenience of having to handle the forms more than once.

OF Form 306 "Declaration for Federal Employment": Applicants should complete and sign/date this form prior to being selected. The supervisor should then review for the applicant’s background prior to hiring. After selected this form must again be signed and dated in Block 16b  as a new appointee.

SF-1199A "Direct Deposit Signup Form": All employees are required to use direct deposit. The bank must complete Section 3 of the form to verify account number and add bank routing number. It is very important that this information be correct. For Section 1 Item C (Claim or Payroll ID Number) put the employees social security number.

SF-85 "Questionnaire for Non-Sensitive Positions"

  1. Begin with this form and follow directions carefully. Fill in all blocks and do not leave blanks.
  2. Pay close attention to item 9 – your employment activities. Begin with the present and work backwards in chronological order. You MUST account for all periods of time for the past five years, whether employed or unemployed. Used the appropriate code and DO NOT leave out a single month. Also, you MUST give supervisors and their addresses, zip and telephone numbers.

SF-87 Fingerprint Chart

  1. Type in all blanks as indicated, or use black ink.
  2. Take to any office having fingerprint equipment, (police dept., etc.)
  3. Both you and the official taking the prints need to sign the form.
  4. Box labeled "Title and Address" refers to the person taking the prints.
  5. Boxes labeled "Position to which appointed" and "Department, etc.," refers to you.

SF-15 "Veteran Preference Claim"

  1. Complete this form if you have had military service and are claiming 10 point preference.
  2. Original, certified, or photostatic copy of discharge certificate and other information regarding dates of entry into military service MUST be attached. These documents will be returned to you when military preferences is approved.
