Please read these instructions before completing any of the following forms.


All New Hires

Send to Human Resources the following information at least two weeks prior to the effective date of your appointment.

OF-306 Declaration of Federal Employment (98kb)
Resume (or OF-612 Optional Application for Federal Employment (269kb)) if not submitted at time of application.
Official College Transcripts: If you are qualifying for your job based on your educational background.
SF-144 Statement of Prior Federal Service (273kb)
SF-181 Race and National Origin Identification (99kb)
SF-256 Self-Identification of Handicap (187kb)
SF-15 Application for 10-Point Veteran's Preference (30kb)

*SF-2817 Life Insurance Election (160kb) due within 31 days of appointment

Medical Exam (will be sent by HR if required)
SF-78 Certification of Medical Exam (Obtain from HR)
