The Self-Assessment Workbook was designed as a tool to help you get a good idea about whether you may be eligible for CSP.

The Workbook will help you:

  • determine eligibility,
  • document stewardship to date, and
  • prepare a benchmark inventory of existing conservation treatments on your land.

Take your completed Self-Assessment and documentation to your NRCS Service Center to determine what category your application fits.  The complete Self-Assessment Workbook is available for on-line use and for download.  For a printed copy or CD-ROM of the Self Assessment Workbook, contact your local NRCS office.


Wisconsin Workbook Application Material   

There are Wisconsin additions to the Self-Assessment Workbook that apply to the signup of land for the CSP sign-up in the Wisconsin watersheds.

Microsoft Word Document.Supplemental Cover Sheet - Collect the items you need from this before scheduling an interview with your NRCS office.

Adobe Acrobat Document.Owner-Operator Intention Sheet - Needs to be completed for rented land that applicants would like to enroll in CSP.

Microsoft Excel Document.Field Information Questionnaire - Complete to determine field eligibility for CSP through the soil conditioning index, a measurement of soil health.  Fields with the same management can be combined on one sheet but must also be listed by field number and tract number and must correspond with field maps included by you.   Here is an example of a completed Questionnaire.

Microsoft Excel Document.Enhancements - Additional conservation treatment options that increase your payments

For Tier II contracts, Excessive Suspended Sediment and Turbidity in Surface Waters of Riparian Corridors must be addressed at some time during the contract.  This could be addressed through conservation buffers for streams and watercourses, if present.
