Self-Assessment Workbook and Wisconsin Workbook Additions

The Self-Assessment Workbook was designed as a tool to help you get a good idea about whether you may be eligible for CSP.

The Workbook will help you:

  • determine eligibility,
  • document stewardship to date, and
  • prepare a benchmark inventory of existing conservation treatments on your land.

Take your completed Self-Assessment and documentation to your NRCS Service Center to determine what category your application fits.  The complete Self-Assessment Workbook is available for on-line use and for download.  For a printed copy or CD-ROM of the Self Assessment Workbook, contact your local NRCS office or call 1-715-832-6547 x 130 to receive one by mail.


Wisconsin Workbook Additions   

There are Wisconsin additions to the Self-Assessment Workbook that apply to the signup of land for the CSP in the Lower Chippewa River Watershed.

Each state has additional worksheets to address their own watershed concerns. 

The following may require Microsoft Excel, Microsoft Word or Adobe Acrobat.

Microsoft Word DocumentCSP 2004 Signup Info - provides lists of information and materials needed for signup.

Microsoft Excel DocumentCrop History and Field Information sheets -  Use these sheets to document cropping practices, rotations, etc.  NRCS will use this to determine the field Soil Conditioning Index, for basic eligibility, funding category placement and, potentially, enhancement payments. Example sheets are also provided - Crop History example and Field Information Example 1 and Field Information Example 2.

Microsoft Excel Document2004 Stewardship Practice List for Cropland and 2004 Stewardship Practice List for Pasture identify the practices and activities in place on the farm and those you are willing to adopt. This is important for determining the appropriate funding category.

Adobe Acrobat DocumentOwner-Operator Intent form Use this for rented land when there is no long-term lease agreement.

Wildlife Habitat Self-Assessments:  Landowners can use the
wildlife self-assessments to determine if land meets the minimum requirement for wildlife in Tier III.
Self Assessments for Declining Species (Select the one that suits your operation best)  Red-hooded Woodpecker   Eastern Meadowlark

The Habitat Suitability Index (HSI's) are completed by NRCS staff during field visits to determine the enhancement level of land that has met the criteria of the self-assessment and could be higher than .5.
